📋 Complete Document Checklist
1. Identification
Original Social Security cards (or clear copies) for every person listed on the return.
Government‑issued photo ID (driver’s license, state ID, passport) for you and your spouse.
Many states now require ID data for e‑file – we encourage all clients to provide it.
2. Income Documents
W‑2 forms from every employer (you and spouse).
1099‑NEC / 1099‑MISC – freelance, contract, or gig work.
1099‑K – if you received payments through third‑party apps.
We cannot file using pay stubs alone unless you’ve exhausted all means of obtaining the W‑2 – call us for help.
3. Investment & Retirement Income
1099‑INT / 1099‑DIV – interest and dividends.
1099‑B – stock, bond, or crypto sales.
1099‑R – pensions, IRAs, 401(k) distributions.
1099‑SSA – Social Security benefits.
1099‑G – unemployment or state tax refund.
4. Home & Mortgage
Form 1098 – mortgage interest.
Closing Disclosure / HUD‑1 – if you bought or refinanced.
Tell us about energy‑efficient upgrades (solar panels, windows) – you may qualify for credits.
5. Education
Form 1098‑T – tuition statement.
Form 1098‑E – student loan interest paid.
Statements from 529 / Coverdell accounts (contributions & distributions).
6. Deductions & Credits
Charitable donations – receipts (written acknowledgment for $250+).
Medical expenses – summary of out‑of‑pocket costs (7.5% AGI threshold).
Property taxes & mortgage insurance – paid receipts.
State/local taxes – if you itemize.
K‑12 educator expenses – up to $300 ($600 if married filing jointly).
7. Business / Rental / Gig Work
Income records (1099‑K, 1099‑NEC, spreadsheets).
Expense records – mileage log, receipts for supplies, home office, equipment.
Vehicle purchase details if used for business.
Rental property income and expense summary.
8. Health Insurance
Form 1095‑A – Marketplace coverage (required).
1095‑B / 1095‑C – helpful but not mandatory.
HSA: Form 1099‑SA and 5498‑SA.
9. Other Situations
Debt cancellation (1099‑C) / Foreclosure (1099‑A)
Gambling winnings (W‑2G) + loss log if itemizing.
Alimony – divorce decree (pre‑2019 agreements).
Estimated tax payments – amounts and dates.
Prior year return – if we didn’t prepare it last year (returning clients: we have it).
10. Special Circumstances
Deceased taxpayer – death certificate & court appointment (if not surviving spouse).
Foreign accounts – if total > $10,000, FBAR may be required.
ITIN application / renewal – original identification documents.
